1. Transfermodel => estafette

De hoofdbehandelaar, huisarts of specialist bepaalt welke professional wordt ingeschakeld en dus eindverantwoordelijk is.
3. Kluwenmodel: stuurloos
Er is geen verantwoordelijk hoofdbehandelaar in de “kluwen”, dus regie is des te meer nodig, waarvoor een casemanager de aangewezen oplossing is.
